Laparoscopic treatment of endometriosis
Endometriosis may affect women in mostly two ways: it can cause pelvic and abdominal pain, and it may be associated with decreased fertility. Although symptoms such as pain and infertility may suggest the possible presence of endometriosis, the only definite way to diagnose is by performing a laparoscopy.
How is the surgery done
Laparoscopy is a surgical procedure performed under general anaesthesia that allows visualisation of the pelvic structures including the uterus, ovaries and the peritoneal surfaces covering the bowel and bladder, where endometriosis is usually found. Laparoscopy also allows for treatment of endometriotic lesions at the same surgical procedure.
What happens during surgery
Surgical treatment for superficial endometriosis (when lesions are just on the surface and don’t go deep into the tissues) can be done by cutting out the lesions ( excision) or by burning the lesions ( ablation).
- Excision of endometriotic lesions or nodules involves cutting out visible areas of endometriosis. When the superficial peritoneal endometriosis lesions are excised, the tissue is sent to the laboratory for evaluation. Surgical excision is also the preferred treatment for ovarian endometriosis (endometriotic cyst) with removal of the cyst wall which allows more preservation of ovarian tissue.
- Destruction of endometriosis is usually by application of heat and is called ablation. Ablation is the treatment of choice for superficial peritoneal endometriosis. This is done by applying intense heat to the areas of endometriosis, the heat being produced by any energy device. Ablation/burning of ovarian tissue for the treatment of endometriotic cysts is not recommended as it is associated with poor fertility outcomes.
- Both excision and destruction/ablation techniques are equally effective with improving pain and fertility in women with superficial peritoneal endometriosis.
